A CHW is a member of your community, who works to make sure you are healthy and accomplishing your goals. They are trained to help you get what you need from Healthcare, Tribal Programs and organizations in your community. CHW’s can help you stay healthy by
helping you get housing, food, transportation and more.
CHW’s can help you stand up for and speak out about your needs and the needs of your community. They work to help you meet your individual/family needs, either on an on going basis or as requested.
Plan and make healthy changes such as:
-Becoming more active
-Eating healthier
-Quit smoking/vaping
Set and reach health goals
Understand and follow care plans from your doctors. For example a CHW can help you find a doctor you like, and ask for additional information to help you understand your illness. A CHW can also help you understand:
-What your health care provider wants you to do
-Understand the details of your conditions and how they may impact your life
Find local services that are tailored to your cultural needs.
-CHW’s help you find information about resources and how to use them
-CHW’s help keep track of the services you choose and follow up with you to see if they are meeting your needs.
Find programs that help with:
-Clothing
-Shelter
-Finances
-Food
-Utility bills
Schedule your healthcare appointments
(your CHW can also attend appointments with you for additional support and advocacy)
